Seriously, what caused it so we can avoid it?

Thanks everyone for the kind words and concerns. The doctors don't know how it happened. They said it typically happens with scar tissue left over from a previous surgery. The explanation was it was probably there since birth.

Oh, that's good to know. Remind me to go back in time and stop myself from being born just in case.
