Vault Assault with joystick 2?

I love this game

 

Great when you want a quick round of frantic action

 

My best is 12,040 so far

 

But can someone explain to me what you're supposed to do when playing with joystick #2

 

I just seem to die immediately, not able to shoot or anything

 

Also, was there another version of this game at some point?

 

The manual seems to suggest this

The second stick's button is used to begin the more-challenging version. You still use the left joystick.
