You just need to wire the parallel cable to a six-pin female header and attach it to the male header on the board in the location and orientation shown in the picture. I built one exactly like this and it works perfectly. The oscillator and all other components in the diagram you posted are already on the SIO2SD board and the power is supplied through the SIO port.

No. This is only good for certain serially programmable types.

 

The old EPROMs were parallel and generally needed higher than standard voltages for the programming process.

 

On top of that, EPROM as opposed to EEPROM needs the UV erase process carried out first.
