Manual Template

Does anyone have a template for creating a manual that would look like one from Mattel Electronics releases "back in the day"?

 

Before I go re-create the wheel, I think it makes sense to ask here, since others have shared box templates in the past.

I have a template I created that is based on Imagic's manuals. They are a bit bigger, but I always liked those better. It's the template I created for Christmas Carol and intend to use it again.
