Does anyone know how to start a 2 player game?

Also, the game has a 32 effect soundbank, but only ever calls 3-4 of them - is there a toggle for this?

There's a trainer version with infinite lives cheats for p1/p2, and you can 'set the cheat' by pausing and pressing f1/f2.... but... how to start a 2 player game?
