Tooth Protectors original 1983 order form

Has the original order form that was used to purchase this game from Johnson & Johnson ever been discovered?  I found several newspaper ads from August of 1983 advertising the game's availability through a mail-in offer with a purchase of three J&J products.  There must've been a display in the health and beauty section with order forms you had to send in.  Does anyone actually have one?

Could be you just wrote a letter. I remember doing that several times for that sort of thing, but unfortunately no lame sounding video games that would one day be worth a mint.

Did someone here manage to win that Tooth Protectors retail display that was on eBay several months back?  From what I remember, it sold for a few hundred dollars.  I was outbid on that one but did manage to grab this full color original circular ad last month.  First time I've ever seen one.  I'll scan and post it sans watermark when I have a chance.

Yes sir...it was a pleasant surprise to see both the display and ad finally surface after all these years.  A higher resolution picture of the display would be nice if someone here won it.

This Johnson & Johnson Tooth Protectors Atari 2600 ad was in newspapers all over the USA on August 7, 1983:

Yes, that's the ad I have which I mentioned above in post #4.  It's in color and very large.

Yep, I posted it for people interested in release dates. Did you ever get a chance to scan and post the color version? I'd like to link to it from the Tooth Protectors entry on the 1983 page.

I didn't...I have several items that I've been meaning to scan.  Part of the problem with this one is that the ad is too large to fit on the bed of my scanner.  I have to find someone with a larger one or scan it in sections and piece them together.
