Good question, Rom.  I've been on your site many times and have often wondered the same thing.  Perhaps my info. isn't completely accurate, but at least it's a start.  Hope this helps.

Centipede Promo Toy - Not completely sure about this one, but I'd guess it's a 2600 item, since the centipede toy resembles the one used in the 2600 art, as opposed to the arcade centipede, which is quite different.  And I believe that "CENTIPEDE - New and only from ATARI" catchphrase was used in conjunction with the 2600 release of the game
