Were these Star Wars rebate offers only done in Canada, or were they done in other parts of the world?

Thanks for sharing this. I have never seen this piece before. Is the English version on the other side of the form?

 

The dates of the rebate are intriguing. According to Random Terrain's list, the game was released in July 1982; I clearly remember playing it at a friend's house later that year (possibly in October). I wonder if sales were flagging by April 1983, hence the rather substantial rebate (worth about $12 today).

Well, here's a rebate coupon from the U.S. with several games, and Empire is one of them.  I hope this answers your question.
