Error 41 when formatting disks

I'm trying to format some floppy disks on the disk drive in my PEB using the Disk Manager 2 cartridge. Unfortunately, I receive a "Disk Error 41" and the format fails.  The light comes on, I can hear the drive spin, I hear the head stepping, but it just doesn't work.

 

Any suggestions?  Googling "TI-99 disk error 41" didn't find anything useful.

That's what I was doing... 40 track, single side, single density.

 

Odd thing is, it's working fine now. Old computers are finicky computers I guess.

Just had this problem and someone suggested checking the indexing sensor on the drive. In my case the bracket holding the sensor had broken and the sensor moved out of position. I secured it back in place and it works fine now.
