These are produced from heavy metal deposits extracted from Nolan Bushnell's joints, producing a signal superior to displayport 1.4.

And here's another from the same vendor named wtf_suckyid, but these are only for lots of two. Don't know where they extracted these materials from.

I've got a couple of those...I think I paid $1.99 for them and thought I was getting ripped off at that price!!!

That's not right, he didn't sell 196 of those at that price. If you look at the seller's sold listings, they've been sold them at NORMAL prices. No idea what's up with the current auctions at ridiculous prices.

who did this cart and how they do it

That is my auction. I originally thought it was done by Sean Kelly also, but after some research over the years I came to believe it was done by Kevin Horton. The BIN price is actually what I paid for it in 2004. It is extremely rare as I have only seen 1 other cart like this since then.

I am pretty sure that it was made by Sean Kelly and it was part of a series of CV MultiCarts (16-in-1, 32-in-1, 64-in-1 and All-in-1) that he developed and released in the late 1990s and early 2000s. I actually saw the exact labeled cart in his store in Norridge, IL, USA that he was using to demo a CV. The version you have up for sale is the All-in-1 and he sold these at CGE as well as on Digital Press. They included a menu program that allowed for game selection via a joystick.

Kevin Horton did develop a MultiCart, but the only one I've ever seen from him was a wire-wrapped version although he may be the person behind the MultiCarts that require dip switch selection.

Here's some pics that I have of the early MultiCarts:
